Hello! I am so excited that you are interested in hormone replacement therapy. Prior to booking a medical evaluation, you need the following:

1. Normal mammogram within in the past 12 months (women only),

2. Lab work (see below for details) and

3. Complete a health questionnaire.

Telehealth medical evaluations are completed on Wed and Thurs evening. If you prefer an in-office evaluation – let us know!

Once the medical evaluation is completed, we will set up your appointment for pellets!
