Short answer: Ozempic and Wegovy are two brand names for the same medication, semaglutide. Ozempic is FDA-approved for type 2 diabetes. Wegovy is FDA-approved for weight management and now comes as a weekly injection (up to 2.4 mg, or 7.2 mg as Wegovy HD) and a daily pill. Same active ingredient, different approved uses, doses and insurance rules.
What is semaglutide?
Semaglutide is a GLP-1 medication. It copies a hormone your gut releases after you eat, which tells your brain you’re full, slows how fast your stomach empties and helps control blood sugar. It’s usually taken as one injection a week, and there’s also a daily pill form. For the bigger picture, start with What is GLP-1?
What’s the difference between Ozempic and Wegovy?
Both are made by Novo Nordisk and both contain semaglutide. The differences are what each is approved for and how high the dose goes:
| Ozempic | Wegovy | |
|---|---|---|
| Active ingredient | Semaglutide | Semaglutide |
| FDA-approved for | Type 2 diabetes in adults (2017) | Chronic weight management (2021), ages 12 and up |
| Forms and doses | Weekly injection, up to 2 mg | Weekly injection up to 2.4 mg; Wegovy HD 7.2 mg (2026); daily pill (approved December 2025) |
| Heart benefit on the label | Lowers risk of heart attack and stroke in adults with type 2 diabetes and heart disease | Lowers risk of heart attack and stroke in adults with heart disease and overweight or obesity |
| Typical insurance coverage | Often covered for diabetes | Varies widely; many plans exclude weight loss drugs |
When people say they’re “on Ozempic for weight loss,” they’re usually using it off-label, meaning for a purpose outside its approved use. The medication is the same, but the dosing plan and the approved purpose differ.
How much weight do people lose on semaglutide?
About 15% of body weight on average at the standard dose. In the STEP 1 trial, adults taking 2.4 mg of semaglutide each week lost an average of 14.9% of their body weight over 68 weeks, compared with 2.4% on placebo. The higher 7.2 mg dose (Wegovy HD) averaged 18.7% in the 72-week STEP UP trial, and the daily Wegovy pill averaged about 14 to 17% in the 64-week OASIS 4 trial. Everyone also followed a reduced-calorie diet and exercise plan, and results ranged widely from person to person.
How is semaglutide dosed?
It starts low and goes up every four weeks, which gives your stomach time to adjust. The standard Wegovy injection schedule steps through 0.25 mg, 0.5 mg, 1 mg and 1.7 mg before reaching 2.4 mg, so it takes about four months to reach that dose. Wegovy HD continues higher for people who need it. Some people stay at a lower dose if it’s working and side effects are easier to manage.
What are the side effects of semaglutide?
The most common are nausea, diarrhea, constipation, vomiting and stomach discomfort, usually strongest in the first weeks and after each dose increase. Smaller meals, stopping when you’re full and drinking plenty of water help. Less common but serious risks include pancreatitis and gallbladder problems.
Semaglutide isn’t right for you if you or a family member has had medullary thyroid cancer or MEN 2, or if you’re pregnant or planning a pregnancy. Read GLP-1 side effects besides nausea for what to watch for.
Is semaglutide or tirzepatide better for weight loss?
Tirzepatide produced more weight loss on average in a head-to-head trial, but that doesn’t make it the right fit for everyone. Tolerance, health history and how your body responds all matter. We compare the two in Tirzepatide vs. semaglutide, and explain tirzepatide on its own in What is tirzepatide?
